    SCHEDULE A - DOCUMENTS TO BE PRODUCED

    Please produce the following records pertaining to the collision that occurred on or about the date referenced in the pleadings between the parties:

    1. Electronic On-Board Recorder (EOBR) and Telematics Data: Any and all GPS, telematics, hard-braking, speed, engine control module (ECM), and routing data generated by the subject commercial motor vehicle for the 24-hour period surrounding the collision.

    2. Driver Hours of Service Logs: Copies of the driver's logs (electronic logging device/ELD or paper) for the thirty (30) days preceding and including the date of the collision.

    3. Dashcam and Video Footage: Any forward-facing, rear-facing, or inward-facing (in-cab) camera footage recorded by the subject commercial vehicle at the time of the collision.

    4. Cell Phone & Data Records: Call logs, text message logs, and data usage records for any cellular device used by the driver of the subject vehicle for the 24-hour period surrounding the collision.

    5. Vehicle Maintenance Records: All repair, maintenance, and inspection records (including pre-trip and post-trip inspection reports and annual DOT inspections) for the subject tractor and trailer for the one (1) year prior to the collision.
